When your doctor prescribes a new medication, one of your first thoughts is probably "Will my insurance pay for this?"

Many people today are enrolled in medical plans in which access to healthcare services is managed in order to keep costs down. These are called managed care plans. One common form of managed care is the health maintenance organization (HMO). An HMO requires patients to see only doctors who are part of the HMO's network. HMO plans usually have lower premiums and charge a modest fee (called a co-payment) per doctor visit.

Other managed care plans include point-of-service (POS) and preferred-provider (PPO) plans, which usually charge a low co-payment for treatment by doctors in the plan's network. The difference with these plans is that they also reimburse patients (at a lower rate) for visits to out-of-network providers.
